Service Directories

Not All Service Directories Are the Same

Patients and families often struggle to find the health care and social services they need. There are many places to look, and many services to sift through. So how do you find the information you need, quickly and effectively?

All directories are not created equal.

If you know what you’re looking for, and you just need the contact information, a phone directory may best suit your needs. Phone directories presents listings with a few data elements – like name, phone number, and address – and they don’t require any expertise or additional guidance in order to find what you need.

If need help for yourself, your loved one, or a patient, but you don’t know what services are available, an Information and Referral (I & R) directory is your best bet. Information is organized by category, so you can browse through different lists to see what is offered, read detailed service descriptions, see if there are eligibility requirements, and learn how to apply for or access the service. I & R directories are available to the public and can also be used by service providers to work with you and find the services you need.

If you have complex needs, or you are supporting someone who does, a System Navigation directory might be your best bet. Accessed on the internet, through live phone services, or in person, a System Navigation directory has all the hallmarks of an I & R directory, and it also includes additional resources like referral forms and comprehensive data sets for larger organizations, like hospitals. Help can be provided anonymously, or you can make a connection with a System Navigator or Care Coordinator who can provide on-going support and follow up.

As our health care and social service systems change, we need to adapt. Navigating these systems effectively is a crucial first step. To learn more, read this white paper: Helping Patients and Families Connect with the Right Services.
